Early Mail and Absentee Ballots are due May 19, 2026, Before 5 PM
Completed early mail and absentee ballots must be received by the District Clerk on or before the day of the vote, Tuesday, May 19, 2026, before 5 PM.

Request for an Early Mail or Absentee Ballot
Please print and complete an application form for an Absentee ballot and return it to the Office of the District Clerk. Ballots will be issued upon the satisfactory completion and submission of the application form. The deadline for filing an Absentee Ballot Application, if the ballot is to be mailed to the resident, is Tuesday, May 13, 2026 at 5:00 p.m. The deadline for filing an Absentee Ballot Application, if the ballot is to be personally delivered to the residents, is Monday, May 18, 2026, at 5:00 p.m. Applications can be sent via mail to: Theresa DeLeva, District Clerk, Patchogue-Medford School District, 241 South Ocean Avenue, Patchogue, NY 11772.

Request for Military Application Military Ballot Applications may be obtained from the Office of the District Clerk and submitted to the District Clerk, by mail (241 South Ocean Avenue, Patchogue, NY 11772), no later than Thursday, April 21, 2026. Military Ballots will be mailed out no later than Friday, April 24, 2026, and must be filed with the
District Clerk by 5:00 p.m. on Tuesday, May 19, 2026.
