Early Mail and Absentee Ballot Applications
Early Mail and Absentee Ballots are due May 20, 2025, Before 5 PM
Completed early mail and absentee ballots must be received by the District Clerk on or before the day of the vote, Tuesday, May 20, 2025, before 5 PM.

Request for an Early Mail or Absentee Ballot
Please print and complete an application form for an Absentee ballot and return it the Office of the District Clerk. Ballots will be issued upon the satisfactory completion and submission of the application form. The deadline for filing an Absentee Ballot Application, if the ballot is to be mailed to the resident, is Tuesday, May 13, 2025 at 4:00 p.m. The deadline for filing an Absentee Ballot Application, if the ballot is to be personally delivered to the residents, is Monday, May 19, 2025, at 4:00 p.m. Applications can be sent via mail to: Theresa DeLeva, District Clerk, Patchogue-Medford School District, 241 South Ocean Avenue, Patchogue, NY 11772.

Request for Military Application Military Ballot Applications may be obtained from the Office of the District Clerk and submitted to the District Clerk, by mail (241 South Ocean Avenue, Patchogue, NY 11772), no later than Thursday, April 22, 2025. Military Ballots will be mailed out no later than Friday, April 25, 2025, and must be filed with the
District Clerk by 5:00 p.m. on Tuesday, May 20, 2025.
