Dear Parent/Guardian:

If you would like to request transportation to a Private or Parochial School for the 2024–2025 school year, please fill out the application below and return it to the Patchogue-Medford Transportation Office no later than April 1, 2024. A separate form is required for each child. Non-public schools must be within fifteen (15) miles or less from the student’s home in order to qualify for transportation via the Patchogue-Medford School District as per Board of Education Policy # 3545.

Patchogue-Medford residents who enroll their students in a non-public school for the 2024-2025 school year must submit their child’s birth certificate (if the child is entering kindergarten in September), demographic information, and documentation of residency to the Central Registration Office. These three (3) original proofs of residency must be submitted in person to the Central Registration Office. The Central Registration entrance is located at the back of Saxton Middle School, 121 Saxton Street, Patchogue.  If you have any questions, Central Registration can be reached by calling them at (631) 687-6430.
