2024-2025 Kindergarten Registration

    Registration for the 2024-2025 school year will be held at 
    Registration Office,
    121 Saxton St., Patchogue, NY 11772
    at the rear entrance.

    Kindergarten registration for the 2024-2025 school year has started! Children turning five years old on or before December 1, 2019 are eligible for kindergarten. Registration appointments will be scheduled after the completion of the online form below. 

  • To register your child for the 2024-2025 school year, an appointment can now be booked online

  • If you have a child who will be entering kindergarten in September 2024, please complete the online form below by Friday, February 9, 2024. You must register even if you are already enrolled in a district pre-school program or UPK.

    Below are the items necessary for kindergarten registration:

    • Kindergarten Registration Packet
    • Home Language Questionnaire 
    • 3 Proofs of residency
    • Birth Certificate
    • Most recent physical exam
    • Most recent immunization records
    • Parent Photo ID
    • Custody/Court Documents (If applicable)

    Our staff can help you determine your child's school based on your address and proofs of residency.

Dual Language Kindergarten Registration Information

  • We have a Dual Language program that is offered to all entering Kindergarten students. The program is housed in four of our seven elementary schools.  If your child is chosen (via lottery) to participate in this program and it is not housed in your home school, the district will provide transportation to the Dual Language School in your zone.

    Kindergarten Dual Language Form

    • Tuesday, June 4th at Eagle Elementary
    • Wednesday, June 5th at Canaan Elementary
    • Wednesday, June 12 at Medford Elementary

     Location: School Library

    • 6:00 pm - 6:45 pm for families that speak English
    • 7:00 pm - 7:45 pm for families that speak Spanish

    These evenings will provide parents with an overview of the benefits of enrolling their child in an educational program that employs instruction in both Spanish and English.
