Under certain circumstances, it may be necessary for a child to receive medication during school hours.  Parents must assume total responsibility for medication and provide a completed medication form signed by both parent and doctor. Forms may be obtained in the Health Office.  A doctor's signature is required for aspirin or over-the-counter medication as well as prescription medication.  Parents are asked to send an adequate supply of medication and update the stock as necessary.  Medication must be labeled with the student's name, date, dose and times to be administered, along with the doctor's name in the original prescription bottle.

Parents must deliver all medication to the nurse for school administration.  Under no circumstances are children permitted to have any medication in their possession including over-the-counter medication.
