School
physical examinations are due in the health office for all
kindergarten, second and fourth grade students.
Also any students new to the district are required to provide proof
of an examination within the school year ( 9-5-07 through 9-30-08
), regardless of grade level.
Our school physician will begin providing exams here in October,
2008.
If your child will be having an examination by their pediatrician
please provide me with a note so I do not schedule them here.
Annual screening for hearing and vision for all students will begin
in the health office in October.

All new kindergarteners must
fulfill NY State immunizations requirements. Those
requirements are:
- 3 polio vaccinations
- 3 DPT (diphtheria, pertussis, tetanus)
vaccinations
- 2 MMR (measles, mumps, rubella) vaccinations
- 3 hepatitis B vaccinations
- Varivax vaccination
**Documentation must be
provided from
physician that student has had or is in progress of receiving these
vaccinations, or the child may not attend school. |

NY State law requires a
physical examination of all children entering the following grades:
- Kindergarten
- Second
- Fourth
- Seventh
- Tenth
*These physicals are to be completed
between September 15, 2007 and September 30, 2008 |

Friendly Reminders:
-
Please notify the
Health Office if your child is coming to school with an injury
sustained at home, requiring a cast, crutches, or Ace bandage.
School policy requires special consideration for these students.

Students who are sick with fever (over 100º)
or have vomiting or diarrhea must stay home until they are
symptom free for 24 hours
