for grades: pre-k, kindergarten, 2nd, 4th, 7th & 10th) Important Information for all parents: Your child's health is of the utmost importance. If a child come to us with a serious injury, or is not feeling well and has a temperature of 100 or more, a call is made for the child to be taken home. It is, therefore, very important that we have up-to-date telephone numbers for the child's home parent's place of work, and two emergency numbers to contact if we cannot reach a parent. If you wish to update your child's emergency card at anytime during the year, please send the new information in writing to the main office.
Kindergarten: If you are registering a child for the current school year, please complete all parts of the health history form. If there is any medical condition that you feel the school nurse should be aware of, please indicated that on the form.
School Physicals: (for grades: pre-k, kindergarten, 2nd, 4th, 7th & 10th) School Examinations are required by New York State Law for grades kindergarten, second, fourth, seventh, tenth and new entrants to the district. These physicals must be done within one year of the start of the new school year and handed in by the end of September. In June, a form is sent home with report card for students entering second and fourth grades in September. The physical form can be returned early September. South Brookhaven Health Center (located on Main Street in Patchogue) offers immunizations free of charge to students without insurance. For further information call the Clinic @ 854-1300. The nurse will continue to do hearing and vision screenings, weights and heights on all students throughout the school year. Also, the 5th grade students will be screened for scoliosis.
Attendance Calls: If your child will be absent or late for school, please call the main office @ 687-6958 before 9 AM.
Gym Excuses: Children who are not feeling well may be excused up to three days with a parent's note. If they are to be excused longer, a physician's note is needed. Please have your child bring all gym excuses and doctor's notes directly to the nurse. Those who are excused from gym will not be able to participate in outdoor recess or field trips.

School nurses and other staff members are not to administer any medication, including all over-the-counter medication to pupils in school.
